Your First Visit

To make your first visit easier, you may complete the Patient Information sheet at home. By clicking on the link below, you can download the form in PDF format from this site. The Authorization to Release Information to a Health Care Provider is only needed if you would like to have records from your previous eye care provider sent to our practice.

 
 
 
 

Appointments

Our office is open Monday through Friday from 8:15 AM to 5:00 PM; we close at 4:00pm on Fridays. Call (919) 467-4500 to schedule an appointment. If you need to cancel or reschedule, please inform the office as soon as possible.

During your visit, we may use eye drops to dilate your eyes. They may briefly blur your vision (especially close range) and make your eyes sensitive to light.

Emergencies

If you have an urgent situation, please call the office immediately at (919) 467-4500. PLEASE DO NOT send an email as that will delay a response and treatment!

Questions

If you have any questions regarding the treatment you are receiving or about the different services Wake Ophthalmology provides, please give us a call and we will be happy to mail you some brochures. In time, these brochures will also become available at this site for download.
